Pharmacological action
Headache is mostly due to active dilatation of blood vessels, as intracranial blood vessels that are very sensitive to the action of histamine, which dilates the cerebral arteries resulting in intense headache,
Ergotamine is the recommended element for the treatment of migraine. Its usefulness is due entirely to its vasoconstrictor action . lt has alpha-adrenergic receptor and serotonin receptor effects.
Caffeine enhances ergotamine vasoconstriction effect and ensures faster and more complete absorption of ergotamine
Meprobamate is a carbamate derivative which has selective effects at multiple stores in the central nervous system including the thalamus and limbic system .It also appears to inhibit multi neural spinal reflexes.lt is a mild tranquilizer which has muscle relaxant properties.

When administered during the prodromal phase of migration it can prevent the development of the vasodilator crisis and during the crisis, it can produce striking relief.
Pharmacokinetics
Absorption of Ergotamine Tartrate from GIT is poor and may be Further decreased by the occurrence of gastric stasis during migraine attack.
Bieaviability is also diminished by the high first-pass hepatic metabolism. Absorption of sublingual ergotamine is very poor.
Caffeine is added to improve absorption of Ergotamine.
Meprobamate is well absorbed from GIT and peak plasma conc. is reached in about1 to 3 hours after oral doses, the elimination half life is ranged from 6 to 17 hours, it is metabolized in Liver and excreted in urine mainly as inactive metabolite
It diffuses across the placenta and appears in breast milk at concentrations of up to 4 times those in the maternal plasma.
Analgin is rapidly hydrolyzed in gastric juice to the active metabolites 4- methyl antipyrine which after absorption undergoes metabolism to 4-formyl antipyrine and other metabolites.
Pentobarbital sodium is well absorbed from GIT after oral doses. It is about 60 to 70 % bound to plasma protein. The elimination half life is dependant and ranged from GIT 15 to 50 hours. It is metabolised in liver and excreted in urine mainly as metabolites.
Indications
Therapy To abort migraine and similar vascular headache attacks.
Dosage and administration
1 to 2 tabets at onset of the attack, then two tablets after a few hours.Initiate therapy at the first sign of the attack.
Do not exceed the recommended dosage.#Narcotic_drugs
